Soukhy Clark, Healthcare Administrator

Soukhy Clark, a servant leader, comes to Peachtree Hills Place with over 20 years in the healthcare industry and has been in various leadership roles for over 15 years. She has served as an Executive Director / Healthcare Administrator in two states for which she and her associates were recognized by the Governor’s Panel of Florida on Excellence in Long-Term Care. The Governor’s Gold Seal was awarded to those communities which demonstrated excellence in long-term care over a sustained period, has promoted the stability of the industry, and facilitated the physical, social, and emotional well-being of the Members.

Soukhy has also served as an Executive Director in various standalone assisted living and memory care communities in Georgia. She is excited to serve as the Healthcare Administrator, overseeing the operations of The Terraces at Peachtree Hills Place and the Health and Wellness Center.

Soukhy holds a Bachelor of Science in Education with a concentration in Recreational Therapy from the University of Memphis in Memphis, Tennessee, Master of Public Administration in Healthcare Administration from Long Island University in Brookville, New York, and she is currently a PhD. candidate in Organizational Leadership from Indiana Wesleyan University in Marion,
Indiana.

Dr. Eleni Footman, Medical Director

Dr. Eleni Footman graduated from undergraduate studies at the University of Florida and medical school at Georgetown University School of Medicine. She completed her residency in Internal Medicine at Inova Fairfax Hospital in Virginia and her fellowship in Geriatric Medicine at New York Presbyterian University/Weill Cornell Medical Center. Her most recently practice experience was at Emory Clinic in Tucker and has served as the Medical Director for the Emory Domiciliary House Call Program.

As a preceptor for Geriatric Fellows, she has provided academic leadership and has provided lectures and facilitated workshops for Emory medical students during Aging Week. She served as a member of the Education Committee at Emory Geriatrics. With excellent references and glowing reports from the patients for which she has served, she will be at Peachtree Hills Place three days a week seeing Members in the clinic or at the Terraces.

Terri Nabritt, MSN, APRN, NP-C, Director, Health & Wellness Program

Terri Nabritt is a Board-Certified Nurse Practitioner currently serving as the Director of the Health and Wellness Program at Peachtree Hills Place. She has over 25 years of experience in the healthcare industry with 20 of those years as a Registered Nurse working in a variety of settings including long term care, critical care, home health, telemetry, medical review, and as a state surveyor. Terri received her Bachelor’s Degree in Nursing from Lander University in Greenwood, SC and her Master’s Degree in Nursing from South University in Savannah, GA. In pursuance of a Doctorate of Nurse Practitioner, Terri has her sights set on Emory University in Atlanta, GA.

Peachtree Hills Place attracted Terri due to a shared passion for detailed, individualized, Member-centered care plans and the exceptional customer service expectation.

Dr. Alexandra Evancho, PT, DPT Physical Therapy

Dr. Alexandra Evancho received her Doctorate of Physical Therapy degree from the University of Alabama Birmingham and her undergraduate honors degrees in Spanish and Psychology from the University of Georgia. She specializes in working with individuals with neurologic diagnoses including stroke, traumatic brain injury, spinal cord injury, and Parkinson’s disease.

She also has experience rehabilitating individuals with both acute and chronic orthopedic diagnoses. She is passionate about providing PT services in the Wellness Center setting and is continuously pursuing further education to ensure that she is the high quality provider that her clients deserve.

Elizabeth “Liz” Perez, Director of Nursing, The Terraces

Elizabeth “Liz” Perez join Peachtree Hills Place as the Director of Nursing at The Terraces. She is excited to join the healthcare team at Peachtree Hills Place. Bringing over 40 years of nursing and leadership experience, Liz enjoys being around the aging population because of their life experiences and wisdom. She looks forward to meeting and providing quality care for the Members of Peachtree Hills Place. We are delighted to welcome Liz to our team.

 

Shenna Scott Heyward, Social Services Director, The Terraces

Shenna Scott Heyward joins Peachtree Hills Place as the Social Services Director at The Terraces. She brings over 35 years of experience. Shenna’s most recent position was providing support and advocacy as the Social Service Director at Powder Springs Transitional Care and Rehabilitation which comprised of 208 bed facility. She enjoys assisting Members with their unique needs and providing community resources as needed. Shenna is the recipient of the Georgia Healthcare Association Social Worker of the Year (1993) and Best Social Worker in Healthcare – Care Stone (2005).

She is delighted to be part of the healthcare team at Peachtree Hills Place. We look forward to having Shenna join the team at The Terraces.
